New Patient Registrations
 
Due to two Partners retiring and two other Partners on maternity leave we are no longer taking on new patients. We are recruiting with the hope to have new Doctors in position in September.
 
You can still register your new born child at the Surgery if you are a Parent, and currently registered at the Practice. You can also register with the Practice if anyone in the same household is already registered at the Surgery and you are living there. Proof of your name and address will be required. 

Register as New Patient

Anyone in England can register with a GP surgery.

You do not need proof of address or immigration status, ID or an NHS number.

Newly registered patients are entitled to an appointment with the nurse for a new patient medical.

The Surgery requires one new patient registration form per patient to be submitted in order for your registration to be processed.

To Complete Your Registration

You will be sent a text message when we have processed the information to inform you have been registered with the practice, the name of your allocated named GP and also a link to follow to complete a new patient questionnaire.

If you wish to have new patient check then please make an appointment to see a Health Care Assistant. 

Once we have accepted you onto our list you will be registered with the Practice and may consult any doctor.  

Every patient is now allocated a named GP. This may be different from the GP you usually see. This does not change the way you book appointments. It means that one GP will have overall responsibility for your care at the surgery.

For continuity of care we encourage patients to try and book appointments with their usual GP. On some occasions this may not be possible so please be assured that all the other doctors that are available on that day will be able to help you with your problem.

Newborn Babies/Children Under 16 Years

Register your baby/child with your GP as early as possible in case you need our help. Come to the surgery and fill in a registration form or simply complete the form via the link below. Please bring the baby's red book, birth certificate or hospital discharge summary as we will need your baby's NHS number.

It's important to register your child with a GP as most children will be unwell at some point and being registered with a GP will make seeing a doctor much quicker and more convenient when you need it. Most parents and carers register their children with their own GP practice.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Disabled Patient Facilities

We will strive to provide access to health screening, assessment and treatment for patients with disabilities, their families and carers.

We will endeavour to provide reasonable adjustments for disabled people, to enable them to access services more effectively, eg:-

  • To facilitate entry there are automatic doors to the front entrances
  • We have wheelchair friendly waiting bays.
  • Allow patients with guide dogs into the Practice.
  • Approach someone directly who is hard of hearing, rather than repeatedly calling their name in the waiting area.
  • Ensuring that the Practice is well lit, particularly after dark.
  • There are disabled patient toilets in both the annex and main building.
